Q:The claim has been rejected due to late submission by priority urgent care. I followed up in July and submitted all the relevant details. But there has been no response from them yet. Q:My mother had some redness in the eye so I took her to an opthalmologist who is in network for United Health PPO. The dr. office charged me a depost of 250 and asked me to claim from the Insurance. How do I proceed with the claim? Do I fill a medical claim form or a vision claim form as the visit was to an opthalmologist for an eye consult?
